Patient Self Requests 

If you are a patient and wish to receive a copy of your own PMP records, please complete the Patient Request form. Email the completed request along with a copy of your driver’s license or valid photo ID to: [email protected]

Patients that have additional questions on how to obtain PMP records, please email [email protected] or call 602-542-2710

Please note, PMP records will only be sent to the patient themselves, we will not send patient records to third-party individuals without a court order.

Compliance

Court Order Submissions/Law Enforcement Requests

In order to release PMP patient records to a third party, a court order is required. The order must be signed by the judge, filed with the clerk, and must direct the Board of Pharmacy to release the PMP records to the requesting entity. 

The patient can also request their own PMP records by completing our patient request form. The form must be notarized and a copy of the patient's driver's license or valid ID is required. We will then release the records to the patient and they can give their records to whomever they choose.
