Dispensing Pharmacist Registration FAQ

The Arizona State Board of Pharmacy Controlled Substances Prescription Monitoring Program (CSPMP) grants access to prescribers and pharmacists so they may review controlled substance dispensing information for patients.

Pharmacists employed by facilities with a valid U.S. DEA registration [1] n must register for the PMP online at https://arizona.pmpaware.net. Effective April 26, 2018, a dispensing pharmacist in an outpatient setting will be required to review the preceding 12-month PMP record of a patient* receiving a schedule II controlled substance at the beginning of each new course of treatment. *A review of a PMP report is not required for veterinary dispensing.

Arizona Revised Statute (A.R.S.) § 36-2606 also requires each medical practitioner licensed under Title 32 and who possesses a DEA license to review the preceding 12 months of a patient’s PMP record before prescribing an opioid analgesic or benzodiazepine controlled substance listed in schedule II, III or IV. Exceptions to reviewing a patient record are described in A.R.S. § 36-2606.

There is no fee to the prescriber or the pharmacist for PMP registration.

To find out whether you are registered in the PMP:

  1. Go to https://arizona.pmpaware.net/,
  2. Click on Reset Password
  3. Enter your email in the login box, and click on Request Password Reset. If you have an account on file, you will be sent password reset information from [email protected] (remember to check your junk mailbox). If you don’t have an account, you will not receive an email.

If you haven’t used your CSPMP account recently, you may need to reset your password. Passwords must be reset every 180 days.

  1. Go online to https://arizona.pmpaware.net/, click on the Reset Password link
  2. Type in your email address
  3. Click the Request Password Reset button
  4. You will receive an email from [email protected] (check your spam and junk mail folders) with information on how to reset your password
  5. If you have a mobile number listed on your account. You can also request a password reset via mobile number.      

Pharmacists who are required to have CSPMP access are those who are employed by or work for a dispensing facility with a U.S. DEA registration number and dispense to patients in Arizona.

To register, go online to https://arizona.pmpaware.net/.

To gain access to the CSPMP, a dispensing pharmacist must use the DEA registration of at least one pharmacy location where the pharmacist works.

Pharmacists licensed in states not interconnected with Arizona’s CSPMP need to register online at https://arizona.pmpaware.net/be sure to select “Out of State Pharmacist” as your role you will need to upload a copy of your valid driver's license, and professional license.

Yes, once a pharmacist is registered to access the CSPMP, the pharmacist may have delegates run patient reports; however, delegates must register for their own accounts. Account sharing is not allowed.

“Pharmacist Delegates – Licensed” (meaning pharmacy interns), “Pharmacy Technicians” and “Pharmacy Technicians in Training” are all eligible to register for the CSPMP under one or more supervising pharmacists. A delegate must be licensed by the state, to be eligible to register.

To register, the delegate will need:

  1. The email address belonging to the pharmacist that the pharmacist uses in the CSPMP
  2. His or her Arizona State Board of Pharmacy license number
  3. Go to, https://pharmacypmp.az.gov/
  4. Click on CSPMP Login
  5. Then, click on “Create an Account” and begin the registration process
  6. Before delegate access is turned on allowing the delegate to run reports on the pharmacist’s behalf, the supervising pharmacist must log in to his or her CSPMP account to approve delegate access. It is the pharmacist’s responsibility to ensure their delegate list is current. For example, if a delegate stops working with the pharmacist, the pharmacist needs to remove that delegate from the pharmacist’s approved list of delegates

For delegates with multiple supervising pharmacists, it is imperative when running a patient report that delegates choose the pharmacist they are working for at the time of the request. This way the correct pharmacist receives credit for checking the CSPMP.
