Providers Must Be Registered With the Prescription Monitoring Program With All Active DEA Numbers

Arizona CSPMP Administrators conduct a biannual audit of DEA numbers registered with the CSPMP in order to identify providers who do not hold active CSPMP accounts. In order to avoid being identified as a provider who is not compliant with the registration requirements outlined in A.R.S. 36-2606, please ensure that any and all active DEA numbers that are in your name are linked to your CSPMP account (including “X” DEA numbers).

If you hold multiple active DEA numbers and not all of them are listed on your CSPMP account, your name may be sent to your Arizona regulatory licensing board for further investigation. It is your responsibility to ensure that all of your active DEA numbers are linked to your CSPMP account.

To verify which DEA numbers are linked to your account, please follow the instructions below:

> Log in to your CSPMP AWARxE Account here: https://arizona.pmpaware.net/login
> Navigate to your name in the top right corner
> Select My Profile
> A list of all DEA numbers associated with your account will be listed under “Profile Info”
> If all of your DEA numbers are listed, nothing further is needed

If you need to add DEA number(s) to your account, please follow the instructions below:

> Complete an Information Change Form here: https://pharmacypmp.az.gov/pmp-registration-info-change-forms
> Select the form for Prescribers & Pharmacists
> Complete the form and include ALL active DEA numbers that are registered in your name, separated by a comma, in the box provided
> Submit the form
> You will receive confirmation in the form of an email from CSPMP staff once your account information has been updated.
