To find out whether you are registered in the CSPMP:

1. Go to https://arizona.pmpaware.net/

2. Click on Reset Password

3. Enter your email in the login box, and click on “Request Password Reset”. The email address you enter should be the email address you believe to be associated with your CSPMP account. If you have an account on file, you will be sent password reset information from [email protected] (remember to check your junk mailbox). If you don’t have an account, you will not receive an email.

 

If you haven’t used your CSPMP account recently, you may need to reset your password. Passwords must be reset every 90 days. 

1. Go online to https://arizona.pmpaware.net/, click on the Reset Password link 

2. Type in your email address associated with your CSPMP account 

3. Click the “Request Password Reset” button 

4. You will receive an email from [email protected] (check your spam and junk mail folders) with information on how to reset your password

If you have a mobile number saved to your CSPMP account, you can reset your password via text. 

1. Select the Reset Password link on the login screen. 

2. Enter your email address associated with your CSPMP account and click continue. 

3. Select the option to receive a text code on your mobile device. Once you select the text message option, you will be sent a text code to your mobile number with instructions on how to reset your password. 

 

No, if you do not have an active DEA registration you are not required nor eligible to have an AZ CSPMP account.

A.R.S. § 36-2606 states if you are a provider with an active AZ practitioner license and possess an active DEA registration, you must have an active AZ CSPMP account.

Yes, A.R.S. § 36-2606 requires a licensed prescriber with an active DEA registration in Arizona to have an account in the CSPMP database. There are no exceptions for certain fields or specialties.

 

No. In 2015, the Arizona Legislature passed Senate Bill 1370 (Laws 2015, Ch. 46) that removed the requirements for Arizona Veterinarians to register with and report dispensing to the Controlled Substances Prescription Monitoring Program. The law went into effect on January 1, 2016.

Once your registration is approved by a CSPMP Admin, your account remains active as long as you have an active AZ  practitioner license AND an active DEA registration. If one or both expire or are relinquished, please contact [email protected] to request your account to be deactivated

Once you have completed and submitted your registration, please allow up to 7 business days for your registration to be approved. Please check your email for further communication from CSPMP Admins regarding any additional documentation that may be needed to complete registration.

CSPMP registrations may be rejected for various reasons. The most common are listed below: 

  • Not verifying your email address within 7 business days of submitting your registration
  • You have an existing CSPMP account that just needs to be updated with current information
  • You did not submit the required documentation to complete your registration (copy of state license and driver’s license for out-of-state prescribers)
  • You did not complete all required fields within 5 days of beginning your registration
  • You are residing/working in a state that AZ currently shares data with and data can be accessed through that state’s PMP 

If during registration you receive a notification that the information you have entered is “already in use”, you may have an existing CSPMP account that needs to be updated with current information. Please complete a change form on our website to have your existing account updated. No. CSPMP accounts must be created by the individual the account is for. Account information such as password credentials can not be shared with anyone.

Yes. If you have multiple DEA numbers, please ensure you have entered all of them during registration.

The system generated a “no reply” email to verify your email address may be marked as spam or junk. Please check both folders for the email. The email link only lasts 20 minutes, if you click on an expired link you will be sent a new email with a new verification link. 

 

 

Medical residents must ensure that the appropriate role is selected upon registering (Medical Resident with Prescriptive Authority). The DEA number used for a medical resident must be the institutional DEA of the facility that the resident is working at along with a suffix that is assigned to the resident by their employer. If you have selected the wrong role upon registering, please contact a CSPMP Admin at [email protected].

Account approval for Medical Residents may take longer than 7 business days. Approval is dependent on the CSPMP Admin being able to verify your resident license on your respective Board’s website.

 

Healthcare specialties are established by CMS. The CSPMP Admins are not able to add or change specialties. Please select the specialty closest to your field of practice. Multiple specialties may be selected. If multiple specialties are selected, the star icon next to the specialty field can be selected and will indicate that specialty is your primary.

Delegates register for their own account, and the provider controls their access by approving the delegate’s access within the provider’s PMP account under User Profile>Delegate Management. Please note it is important to remove a delegate that's approved under your supervision once that delegate no longer works for you.
